3. Found an eligible patient? Then generate study number and print off required documents.

4. Consent patient.

5. Complete questionnaire A to retain. Ask patient to complete questionnaire B for posting to Leeds at 6-month intervals.

6. Collect 10mls of blood (plain tube) for serum to be frozen and mailed to Rotterdam in batches at 6-month intervals.

7. Take 20ml (EDTA tube) blood sample (DNA). Freeze and post to Leiden in batches at 6-month intervals.

8. Complete the clinical & pathological details via the online database.

---

9. Obtain pathology blocks & slides for review and postage to Leeds.

---

10. Administer annual follow-up questionnaire F by telephone.

---

11. Complete relapse data online annually unless the patient is part of adjuvant EORTC trials.
